To date, physicians have treated the disease with anything that appears to help. Surgery has been the only predictable approach to treatment but has been reserved for severe cases where deformity and/or pain prevent sexual intercourse or when no other option has succeeded. Complications can develop from surgery, including a shortening of the penis. Additionally, Some patients who elect surgery report that the disease returns after a period of time.

The non-surgical approaches to treatment to date have been generally been ineffective. Attempts to dissolve the plaque or scar tissue by direct intra-lesional injections have been tried since the late 1800s. The injections have been with mercury, iodine, fibrinolysins, steroids, verapamil and clostridial collagenase. These techniques have offered very limited success. The problems associated with direct intra-lesional injections is the density or hardness of the plaque. It is difficult to uniformly inject medication throughout a very dense plaque. Since the plaque consists of collagen, one would think that collagenase injections would offer the best result. However, the density of the plaque remains a concern, additionally the collagenase can dissolve healthy tissue as well as diseased tissue.

External beam radiation and intra-lesional implantation of radium seeds have been tried. Some authorities have seen some degree of success if radiation is used in the early stages of the disease process, and they claim that radiation has been successful for relieving pain but ineffective for disfigurement.

Heat treatments using diathermy as a heat source have been used. Low voltage electrical devices have been developed for use in the home whereby an electric current is used to help "drive" a medication into the plaque. Lasers have also been used to "vaporize" the plaque, but again with questionable success.

To date there has not been a truly effective treatment for Peyronie’s Disease, a disease that often produces such severe discomfort and distress that sufferers have been willing to endure such treatments as penile injections.
